Decreased AED levels during pregnancy occur due to increased drug clearance and some degree of haemodilution. However, the decrease in total levels of AEDs during pregnancy is counterbalanced by an increase in the percentage of free (unbound by protein) fraction and therefore bioactive fraction of AEDs due to decreased plasma protein binding and decreased albumin concentration during pregnancy 17 . Therefore, the free fraction, which is an indication of brain concentrations, often remains stable, preventing the need for dose adjustment. If possible, for drugs that are highly protein bound (the older AEDs valproate, and phenytoin as well as the new AED tiagabine), free levels rather than total levels should be obtained pre-pregnancy, and this is the value that should be maintained during pregnancy. It is not necessary to maintain total levels at pre-pregnancy rates, and attempting to do so may in fact cause toxicity and unnecessary exposure to higher levels in the fetus.
